Karoshi is one of the few Japanese words that entered English because the thing it named had no equivalent word elsewhere. It means death caused by overwork, and it covers two things: strokes and heart attacks brought on by long hours, and suicide caused by work, which is sometimes called karo-jisatsu. It is not a figure of speech.
It is an administrative category, with guidelines saying that around eighty hours of overtime a month over several months indicates a link to the work. In the 2024 fiscal year a record 1,304 claims were approved, and for the first time over a thousand of them were for mental illness rather than physical collapse.
Japan has responded: a 2014 act made prevention a state responsibility and requires an annual white paper, and from 2019 overtime has a legal ceiling of forty-five hours a month in principle. Hours have fallen. Whether the culture behind them has changed is a separate question, and one Japanese people argue about.

「過労死」という語は1970年代末から使われ、1980年代の相談活動を通じて社会に広まった。英語でもそのまま karoshi として辞書に載っている。平成26年（2014年）に過労死等防止対策推進法が成立し、国に防止対策と年次報告が義務づけられた。平成31年（2019年）からは時間外労働の上限が法律で定められ、原則として月45時間・年360時間、特別の事情があっても年720時間などの枠が設けられた。令和6年度（2024年度）の労災認定は1304件で過去最多、うち精神障害が1057件だった。
