The okusuri techo is a booklet in which pharmacies record each prescription: the drug, dose and date. You bring it to every pharmacy and hospital so that interactions and duplicates are caught. It proved its worth after disasters, when evacuees could show what they were taking.

In Japan, medicines are usually dispensed not at the hospital but at a separate pharmacy, and each time you collect a prescription the pharmacist adds a sticker to your okusuri techo, a small booklet recording what you were given, the dose, and the date. You carry it to every appointment.
The purpose is practical: a pharmacist seeing the full list can catch a dangerous interaction between drugs prescribed by different doctors, or spot that two doctors have prescribed the same thing. It also lowers the dispensing fee slightly if you bring it.
Its most quoted justification came after the 2011 earthquake, when people who had evacuated without their medicines could show the book and be given the right replacements, and pharmacists have cited that experience ever since. Smartphone versions now exist and are accepted at many pharmacies. For a foreign resident it is genuinely worth keeping, especially if you see more than one clinic.
日本では、薬は病院ではなく別の薬局で受け取ることが多く、処方を受け取るたびに薬剤師がお薬手帳にシールを貼ります。渡された薬、量、日付が記録された小さな手帳です。受診のたびに持っていきます。目的は実際的なものです。
一覧をまとめて見られる薬剤師は、別の医師が出した薬どうしの危険な飲み合わせに気づけますし、二人の医師が同じ薬を出していることも見つけられます。持参すると調剤の料金が少し下がる仕組みもあります。最もよく挙げられる根拠は2011年の震災のあとに生まれました。
薬を持たずに避難した人が手帳を見せることで、同じ薬を受け取ることができたのです。薬剤師はその経験を今も語ります。携帯電話の中に記録する形も広まり、多くの薬局が対応しています。在住の外国人にとっても、とくに複数の診療所にかかるなら、持っておく値打ちが本当にあります。

どうして薬局は病院と別なのですか。
Policy since the 1990s has separated prescribing from dispensing, so doctors prescribe and independent pharmacists dispense and check. That is why there is a cluster of pharmacies outside every large hospital.
1990年代以降の政策で、処方と調剤を分ける医薬分業が進められてきました。医師が処方し、独立した薬剤師が調剤して確認します。大きな病院の前に薬局が並んでいるのはそのためです。

お薬手帳はどうすればもらえますか。
Ask at any pharmacy and they will issue one free. If you already have one and forget it, the stickers are given to you loose so you can attach them later.
どの薬局でも頼めば無料で作ってくれます。持っているのに忘れた場合は、シールを渡してくれるので、あとで貼ることができます。
QCan I bring my own medicines from abroad?
外国から自分の薬を持ち込めますか。
Limited quantities for personal use are generally allowed, but some common foreign medicines contain ingredients restricted in Japan. Check the health ministry's rules before traveling and carry a doctor's note.
個人で使う分の一定量はおおむね持ち込めますが、外国でふつうの薬にも、日本で規制されている成分が含まれていることがあります。渡航の前に厚生労働省の案内を確かめ、医師の証明書を携帯してください。
