Every Japanese school has a hokenshitsu, staffed by a full-time nurse-teacher called a yogo kyoyu. It handles scraped knees, headaches and health checks. But it also serves a second purpose that surprises people: a student who cannot cope with the classroom may be allowed to spend the day there instead of going home, and the nurse is often the adult they will talk to.

Every school in Japan has a room called the hokenshitsu, with a qualified nurse-teacher in it all day. The obvious function is first aid and health checks. The less obvious one matters more. If a student cannot manage the classroom, because of bullying, anxiety, family trouble or something they cannot name, many schools allow them to attend school but spend the time in the health room.

It is called hokenshitsu tokou, attending via the health room, and it is a recognized arrangement rather than a loophole. The reasoning is that a child who is in the building is still connected to school, and that losing the habit of coming at all is much harder to recover from.

The nurse-teacher ends up being, for some students, the only adult at school they will speak to honestly, because that person gives no grades and runs no class. Whether this is a good solution is debated, and it clearly depends on the individual nurse. But it exists, and it is a piece of Japanese schooling that outsiders rarely hear about.

日本の学校には保健室という部屋があり、資格を持った養護教諭が一日います。分かりやすい役目は手当てと健康診断です。分かりにくいほうの役目のほうが大きいかもしれません。いじめ、不安、家庭の事情、言葉にできない何か。

教室にいられない生徒に対して、多くの学校は登校はして保健室で過ごすことを認めています。保健室登校と呼ばれ、抜け道ではなく認められた形です。学校にいれば学校とのつながりは切れない、来る習慣そのものを失うと戻すのがずっと難しい、という考え方です。

養護教諭は、一部の生徒にとって、学校で本音を話せる唯一の大人になります。成績をつけず、授業も持たない立場だからです。これがよい解決かどうかは議論がありますし、その先生次第であることも明らかです。それでもこの仕組みはあり、外の方にはあまり知られていない日本の学校の一面です。

QCan a student really stay there all day?

生徒は一日中そこにいてもいいのですか。

In many schools, yes, with the school's agreement. It is treated as attendance rather than absence, which matters for the student's record and for keeping them connected to school.

学校が認めれば、多くの学校で可能です。欠席ではなく出席として扱われます。記録の面でも、学校とのつながりを保つ意味でも、そこが大事です。

QIs there a school counselor too?

スクールカウンセラーもいるのですか。

Many schools now have one, though often only on certain days. The health room nurse is there every day, which is part of why students go to that room first.

豆知識

養護教諭は学校教育法に基づいて置かれる教員で、一定規模以上の学校には配置が義務づけられている。担任や教科を持たない立場であることが、生徒にとって話しやすさにつながっているとされる。保健室登校は文部科学省の調査でも扱われる区分で、不登校の状態にある子どもが学校とのつながりを保つ形のひとつとして位置づけられている。なお日本の学校では、年度のはじめに身長・体重・視力・聴力などの健康診断が全員に対して行われ、その記録の管理も保健室が担う。
