But it also serves a second purpose that surprises people: a student who cannot cope with the classroom may be allowed to spend the day there instead of going home, and the nurse is often the adult they will talk to.

Every school in Japan has a room called the hokenshitsu, with a qualified nurse-teacher in it all day. The obvious function is first aid and health checks. The less obvious one matters more. If a student cannot manage the classroom, because of bullying, anxiety, family trouble or something they cannot name, many schools allow them to attend school but spend the time in the health room.
It is called hokenshitsu tokou, attending via the health room, and it is a recognized arrangement rather than a loophole. The reasoning is that a child who is in the building is still connected to school, and that losing the habit of coming at all is much harder to recover from.
The nurse-teacher ends up being, for some students, the only adult at school they will speak to honestly, because that person gives no grades and runs no class. Whether this is a good solution is debated, and it clearly depends on the individual nurse. But it exists, and it is a piece of Japanese schooling that outsiders rarely hear about.
