What surprises visitors is how ordinary it is: there is no separate license, a regular doctor can prescribe it, over a hundred formulas are covered by national health insurance, and the granules come in the same kind of paper sachet as any other medicine.

If a Japanese doctor hands you a sachet of brown powder for a cold, that is kampo. It is herbal medicine that came from China more than a thousand years ago and then developed separately here, so it is related to Chinese medicine but not the same: Japanese practice settled on a fixed set of formulas with fixed proportions, and leans on what the patient's body presents rather than on pulse and tongue reading to the same degree.
The striking thing for a foreign patient is how mainstream it is. There is no separate profession; the same doctor who writes you an antibiotic can write you a kampo formula, it is taught in medical schools, and well over a hundred formulas are covered by national health insurance.
Acupuncture, by contrast, does need its own license and usually its own clinic. The most famous formula is a cold remedy taken at the first shiver, and half the country keeps a box of it at home.
風邪をひいて、日本の医師から茶色い粉の分包を渡されたら、それが漢方です。千年以上前に中国から伝わり、その後この国で別に発展した生薬の医療で、中医学とは親戚ですが同じではありません。
日本では、配合の決まった処方の組に落ち着き、脈や舌を見ることよりも、患者の体に現れているものを手がかりにする度合いが高くなりました。外国の患者が驚くのは、これがどれほど主流にあるかです。別の職業ではありません。
抗生物質を出すのと同じ医師が漢方を出しますし、医学部でも教えます。百を超える処方に健康保険が使えます。一方で鍼灸には別の資格が要り、たいていは別の治療院で受けます。もっとも知られた処方は、寒気がした最初の段階で飲む風邪薬で、家に箱を置いている人は珍しくありません。

QIs it the same as Chinese medicine?
中国の漢方と同じものですか。
Related but not identical. Both come from the same classical texts, but Japan standardised a limited set of formulas at fixed doses, while Chinese practice adjusts ingredients patient by patient. The Japanese doses are generally lighter.
近いけれど同じではありません。もとの古典は共通ですが、日本は限られた処方を決まった分量で標準化し、中国では患者ごとに生薬を加減します。日本の分量は全体に軽めです。
QCan I get it without seeing a doctor?
医師にかからずに買えますか。
Yes. Drugstores sell many formulas over the counter in the same aisle as ordinary cold and stomach medicine, usually in smaller doses than the prescription version. The packaging is in Japanese, so ask the pharmacist.
買えます。薬局では多くの処方が、風邪薬や胃薬と同じ棚に市販されています。分量は処方されるものより少なめです。表示は日本語なので、薬剤師に相談してください。
QDoes it actually work?
本当に効くのですか。
It depends on the formula. Some have solid clinical evidence and are used routinely in hospitals, for example for post-operative bowel problems; others rest mainly on long use. Ask what it is for and whether it interacts with anything you already take.
処方によります。臨床で裏づけがあり、手術後の腸の不調などに病院で日常的に使われるものがある一方、長く使われてきたという根拠が中心のものもあります。何のための薬か、今飲んでいる薬と重ならないかを確かめてください。
