Kenko-shindan is the health checkup almost every full-time employee in Japan goes through once a year, and it is not optional in the way an annual physical might be elsewhere. The Industrial Safety and Health Act requires employers with regular staff to provide it, on the company's time and the company's bill, and to report the results to the local labor office if the workforce is large enough. The exam itself is fairly standardized and quick: blood pressure, blood and urine tests, a chest X-ray, height and weight, an eyesight and hearing check, sometimes done at a clinic and sometimes from a mobile van that parks outside the office for a day.
From age forty there's an additional layer, a metabolic-syndrome-focused checkup called tokutei kenshin that specifically measures waist circumference, because Japan treats abdominal fat as an early-warning sign worth tracking nationally. The one part that catches foreign residents off guard is the barium test some clinics still use to check the stomach, where you drink a chalky liquid and are rotated on a table while X-rays are taken.
