Hito is a word you will see in Japanese travel writing, meaning a hidden or secret hot spring. It is not an official category. It was popularized by a group of inns that banded together in the 1970s to market remote hot springs, and it has been a useful marketing word ever since. That said, the places are genuinely remote.
A typical hito has one inn, a bath filled directly from the spring, no convenience store, no mobile signal, and a bus that runs twice a day if at all. Some close entirely in winter because the road is not cleared. The appeal is that nothing has been added: no music, no vending machines, no menu.
For visitors this can be either the best night of a trip or a difficult one, so it is worth being honest about what you are choosing. If you want a hot spring with an English-speaking front desk and a late dinner, a hito is the wrong choice. If you want to hear nothing but water for twelve hours, it is exactly right.

Often not. Bring the reservation written down, learn a few phrases, and expect meals to appear without being ordered. A translation app works if there is any signal, which there may not be.

QIs the water different from a normal onsen?
普通の温泉と湯は違うのですか。
Not necessarily, but hito inns often pour spring water straight through the bath without recirculating it, which is called gensen kakenagashi. Whether that suits you depends on the mineral content more than the location.
必ずしも違いませんが、秘湯の宿は湯を循環させず源泉をそのまま流す源泉かけ流しであることが多いです。体に合うかどうかは、場所より泉質によります。

FACTIt is a marketing word with no legal or official meaning, popularized from the 1970s. The remoteness is usually real, but the label is not a certification.

秘湯という言葉は、1975年に山あいの温泉宿が集まって結成された団体の名に使われたことで広まったとされる。公的な分類ではないため、どこまでを秘湯と呼ぶかに決まりはない。交通の便が悪いことが条件のように語られるが、道路が整備されて行きやすくなった宿も多い。冬季に休業する宿、自家発電で電気をまかなう宿、湯を引くために長い樋を山から渡している宿もある。なお「源泉かけ流し」は湯を循環・再利用しない方式を指す言葉で、秘湯かどうかとは別の話である。
