A handful of newer facilities break from this: Hakone Kowakien Yunessun, for example, requires swimsuits in its water-park zone but stays nude-only in its separate traditional bathing area. Visitors who assume covering up is the polite choice often find it's actually the opposite.

Ask a first-time visitor to guess the polite way to enter a Japanese bath, and many assume keeping a swimsuit on shows modesty.
In fact, the opposite is closer to true: at a traditional onsen or sento, everyone bathes completely nude, and bringing in a swimsuit is treated as a hygiene issue rather than a matter of manners, since fabric holds soap, sweat and body oil that the shared water is never meant to filter out. This isn't universal, though.
A small number of facilities are built the other way around. Hakone Kowakien Yunessun, a hot spring amusement park, requires swimsuits in its colorful water-park zone, complete with slides and a coffee-scented bath, while keeping a separate, traditional nude bathing area next door. Knowing which kind of facility you're at saves an awkward moment at the changing room door.

It's a separate issue — nudity is about hygiene, while many facilities restrict visible tattoos for historical reasons connected to organized crime, and the two rules can apply independently of each other.
