Sports Day in Japan falls on the second Monday of October, and it carries a fairly direct piece of history. The 1964 Tokyo Olympics opened on October 10th, and two years later that date became a public holiday called Health and Sports Day. For Japan, the 1964 games were the moment the country presented itself to the world again after the war, alongside the first bullet train, which opened days earlier.
So the holiday was, in effect, an anniversary of that. Then it was detached from its own date: from 2000 it moved to the second Monday of October to make a three-day weekend, so it now rarely falls on the tenth. In 2020 it was renamed Sports Day, dropping the word for physical education, in the run-up to the second Tokyo Olympics.
That renaming is a small sign of a wider shift, from sport as something schools make you do to sport as something you choose. School sports days do cluster in this season, which is why people associate the two, but a school's own event is usually on a different day.
日本のスポーツの日は10月の第2月曜日で、はっきりした来歴があります。1964年の東京オリンピックは10月10日に開会し、その2年後、この日が体育の日という祝日になりました。日本にとって1964年の大会は、戦後ふたたび世界に向けて自らを示した瞬間で、その数日前には新幹線も開業しています。
つまりこの祝日は、その記念日でした。ところが、自分の日付から切り離されます。2000年から10月の第2月曜に移り、三連休になったため、いまは10日に当たることはめったにありません。そして2020年、二度目の東京オリンピックを前に、体育という語を外してスポーツの日に改称されました。

The old name used taiiku, the word for physical education, which carries a school and discipline connotation. Sports Day was adopted in 2020, timed with the Tokyo Olympics, and reflects a shift toward sport as recreation rather than schooling.
旧称の体育は学校教育と規律の含みを持つ語です。2020年に東京オリンピックに合わせてスポーツの日となり、学校で課されるものから、楽しむものへという流れを反映しています。

Often near it, but usually not on it, since the holiday itself is a day off. Many schools now hold theirs in May or June instead, partly because October is still hot in much of Japan.
近い時期が多いですが、当日ではないのが普通です。祝日は休みだからです。いまは5月や6月に移した学校も多く、10月でもまだ暑い地域があることが理由のひとつです。

Yes. It was shifted to the opening day of the Tokyo games in 2020 and again in 2021 when they were postponed, by special legislation. It has since returned to the second Monday of October.

1964年の東京オリンピックの開会式が10月10日に行われたことにちなみ、1966年に体育の日が国民の祝日として定められた。2000年のいわゆるハッピーマンデー制度で10月の第2月曜日に移り、2020年にスポーツの日へ改称された。改称は2018年の祝日法改正によるもので、2020年の東京オリンピックを見据えたものとされる。なお2020年と2021年は、大会の開会式に合わせて日付が特例で移動した。東京オリンピックの開会に先立つ1964年10月1日には東海道新幹線が開業しており、この二つは戦後復興の象徴として並べて語られることが多い。
