A maid cafe is a themed cafe where the staff dress as maids and treat you as the master of a household returning home. It grew out of Akihabara around the start of the 2000s, first as a niche for anime fans and then as something tourists came to see.
The service is a performance: a greeting at the door, a short ritual over your food, sometimes a song or a game for an extra fee. The important thing to understand is the boundary. It is not a bar, it is not adult entertainment, and the rules are strict on both sides.
You may not touch the staff, you may not photograph them without paying for an arranged photo, and you may not ask personal questions or wait for them outside. Within those rules it is simply a cafe with a script, and the bill is higher than the coffee is worth.

Most shops add a seating charge per person per hour on top of food and drink, and extras like a photo or a song are charged separately. Check the board at the entrance before you go in, because this catches visitors out constantly.
