「バイリンガルは認知症が4年遅れる」は本当? 話題の研究とその後の反論

病院の記録では、2言語を話す人は認知症の症状が出る年齢が4年ほど遅かった。ただ、追跡調査では「なりにくい」とまでは言えず、議論が続いている。
ひとことで言うと2つの言葉を話すと脳にいいかも。でも「認知症を防ぐ」とはまだ言えない!
「2つの言語を話す人は、認知症になるのが4年以上遅い」。
2007年、カナダの研究チームが発表したこの結果は、世界中のニュースで取り上げられました。「英語を勉強すれば認知症予防になる!」と受け取った人もいたかもしれません。
でも、その後の研究を追っていくと、話はもう少し複雑でした。「発症が遅い」と「なりにくい」は、まったく別の話だったのです。
012007年:症状が出るのが約4年遅かった
インドのハイデラバードで行われた研究(アッラディら、2013年)でも、認知症患者648人の記録で、2言語以上を話す人の発症が4.5年遅かったと報告されました。この研究では、読み書きができない人の間でも同じ傾向が見られ、「教育のおかげ」だけでは説明しにくいとされました。
02追跡研究では「なりにくさ」に差がない
ところが、別のタイプの研究では違う結果が出ています。
上の2つは、すでに認知症と診断された人の記録をさかのぼる後ろ向き研究。一方、健康な人を何年も追いかけて、誰が認知症になるかを見る前向き研究では、結果が分かれました。
ロンドン大学のムカダムらは2017年、前向き研究をまとめたメタ分析を発表しました。約5,500人の参加者のデータで、認知症になるオッズ比は0.96(ほぼ差なし)。バイリンガルが認知症を防ぐという証拠は見られなかったのです。
2020年のブリーニらのメタ分析も、「発症は遅いが、なるリスクは下がらない」と結論しています。

03なぜ結果が分かれるのか
ひとつの考え方は認知予備力です。脳に変化が起きても、2言語を使い分けてきた経験などで症状が表に出るのを遅らせているのかもしれない、というもの。これなら「発症が遅い」と「なるリスクは同じ」は矛盾しません。
一方で、移民かどうか、教育、職業、受診のタイミングなど、言語以外の違いが結果をゆがめている可能性も指摘されています。後ろ向き研究では「いつ症状が始まったか」を家族の記憶などに頼ることが多く、その正確さにも限界があります。
さらに、「バイリンガルは注意や切り替えの力が高い」という脳の利点そのものについても、成人152研究をまとめたレトネンらのメタ分析(2018年)で、出版バイアスを補正すると利点は確認されなかったと報告されています。
04まとめ
「バイリンガルは認知症が遅れる」は、半分ホント。病院の記録では症状が出る年齢が遅いという結果がくり返し出ていますが、認知症になること自体を防ぐとは言えません。
英語を学ぶ理由は、脳のためだけでなくていい。世界が広がる楽しさそのものが、一番の価値です。

In 2007, Bialystok and colleagues reported that among 184 patients at a Toronto memory clinic, lifelong bilinguals had developed dementia symptoms about four years later than monolinguals. The finding drew attention worldwide. A 2013 study of 648 patients in Hyderabad, India, found a delay of 4.5 years.
The delay held even among patients who could not read or write, so education alone was unlikely to explain it. Both studies, however, were retrospective. They examined people who had already been diagnosed and often relied on relatives' memories to date when symptoms began. Prospective studies follow healthy people to see who develops dementia, and they tell a different story.
A 2017 meta-analysis including about 5,500 participants found an odds ratio of 0.96, which is essentially no difference. A 2020 meta-analysis concluded that bilingualism is linked to later onset but not to lower risk. Clinic-based findings may also be distorted by immigration status, occupation and the timing of clinic visits.
Even so, the two sets of results need not conflict. Under the cognitive reserve hypothesis, years of switching between languages might help the brain cope with damage for longer. That would push back the first symptoms without lowering the risk of disease.
