虫垂(盲腸)は「進化の名残」じゃないかもしれない。腸内細菌の"避難所"説と、手術しない治療

虫垂炎(いわゆる盲腸)は、必ず手術で虫垂を取らないと治らない

虫垂はムダな器官とされてきたが、腸内細菌の「避難所」として働く説が提案され、哺乳類で少なくとも2回独立に進化したことも示された。虫垂炎は抗菌薬で治せる例もある。
ひとことで言うと盲腸の先の虫垂は、腸の良い細菌を守る場所かもしれない。
虫垂(ちゅうすい)は、大腸の始まりにある盲腸から飛び出した、長さ5〜10cmほどの細い袋です。
「いわゆる盲腸」として知られ、炎症を起こすと手術で取ります。取っても困らないので、長いあいだ「進化のムダな名残」の代表例として教科書に載ってきました。
ところが21世紀に入って、この見方が少しずつ変わってきています。
01「独立に進化した」という手がかり
進化のムダな名残なら、ふつうは消えていく方向に進みます。ところが――
独立に生まれ、長く残っている。これは「ただのゴミ」ではなく、何かの役に立っていることを示す有力な手がかりです。
02腸内細菌の「避難所」説
虫垂には、リンパ組織(免疫の組織)がたくさん集まっています。この点をヒントに、2007年に新しい仮説が提案されました。
抗菌薬や衛生環境が整う前の時代には、下痢で腸内細菌を失うことが命に関わった可能性があります。避難所があれば有利だったかもしれません。

03間接的な証拠:再発しにくい?
この説を支持する臨床データもあります。
2011年、米国の研究チームは、クロストリジオイデス・ディフィシル感染症(抗菌薬の使用後などに起こる腸の感染症で、再発しやすい)の入院患者の記録を調べました。すると、虫垂がある人のほうが再発が少ないという関連が見られました(多変量解析で調整後の相対リスク約0.40)。
ただしこれは後ろ向きの観察研究で、因果関係の証明ではありません。研究者自身も、前向きの研究で確かめる必要があると述べています。
04どんでん返し:虫垂炎は「必ず手術」ではなくなってきた
もう1つの大きな変化が、治療法です。
05「ムダな器官」という言葉の落とし穴
虫垂の話は、生物学の用語の難しさも教えてくれます。
進化で言う「退化した(vestigial)」は、「元の機能を失った」という意味で、「何の役にも立たない」とは違います。虫垂は、消化の機能はほとんど失っていても、免疫や細菌の維持という別の役を担っている可能性があるのです。

The appendix, a narrow pouch about 5 to 10 centimeters long, has often been described as a useless leftover of evolution. However, in 2009, a team led by Smith compared many mammals and found that the appendix had evolved independently at least twice and had been kept for more than 80 million years. Such a long history suggests that it may have a function.
One idea, proposed in 2007, is that it acts as a "safe house" for helpful gut bacteria. If an illness flushes bacteria out of the intestine, those hiding in the appendix could help the gut recover. Meanwhile, a large 2020 trial showed that many patients with appendicitis could be treated with antibiotics instead of surgery.

Textbooks long presented the appendix as a classic vestigial organ, a remnant of evolution with no purpose. That view has weakened. In 2009, Smith and colleagues mapped the appendix across many mammal species and concluded that it had evolved independently at least twice and had persisted for more than 80 million years, which is hard to explain if it were useless. Bollinger's group proposed in 2007 that the appendix serves as a safe house for beneficial gut bacteria, which form biofilms in its narrow, dead-end shape and could repopulate the intestine after severe diarrhea.
Indirect support comes from a 2011 study in which patients with an appendix had fewer recurrences of a difficult intestinal infection, although this retrospective association does not prove cause and effect. Treatment of appendicitis has also changed. In the 2020 CODA trial involving 1,552 adults, antibiotics were not inferior to surgery in quality of life at 30 days, yet 29 percent of the antibiotic group needed surgery within 90 days, and complications were higher in patients with a hard deposit in the appendix. "Vestigial" means an organ has lost its original role, which is different from having no role at all.
