「平熱は37℃くらいでしょ?」
体温計で36.2℃と出て「低すぎるかな」と心配したり、37.0℃で「微熱だ」とあわてたり。私たちは、なんとなく「37℃」を基準に考えがちです。
でも、この37℃という数字、実は150年以上前の研究がもとになっていました。そして最新のデータは、少し違う姿を見せています。
0137℃は19世紀ドイツの医師の数字
「平熱37℃(華氏98.6度)」を広めたのは、19世紀ドイツの医師ヴンダーリッヒです。彼は大量の患者の体温を測り、健康な人の平均を37℃としました。体温を「病気のサイン」として使う考え方を広めた、医学史の重要人物です。
ところが1992年、米国メリーランド大学のマコウィアックらは、学術誌『JAMA』で「98.6°F(37℃)を批判的に見直す」という論文を発表しました。健康な成人を調べると、平均は37℃より低かったのです。
023万5千人のデータでは平均36.6℃
さらに2019年の系統的レビュー(ジェネバら)では、36本の論文・7,600人以上のデータをまとめたところ、測る場所によって平熱の範囲が違うことが改めて示されました。たとえば、わきの下は口の中より低めに出ます。60歳以上の人は、60歳未満より平均0.23℃低いこともわかりました。
03150年で、体温が下がってきた?

もっと驚きの研究もあります。2020年、米国スタンフォード大学のプロツィフ(Protsiv)らは、
- 南北戦争の北軍退役軍人の記録(1860〜1940年に測定、約2万4千人)
- 1970年代の国民健康栄養調査(約1万5千人)
- 2007〜2017年のスタンフォード大学の医療データ(約15万人)
という、時代の違う3つのデータを比べました。すると、生まれた年代が10年新しくなるごとに、体温が約0.03℃ずつ下がっていたのです。
原因ははっきりしていませんが、研究チームは、感染症や慢性的な炎症が減ったこと、暖房や冷房で体温調節の負担が減ったことなどで、体の代謝が変わった可能性を挙げています。昔の「37℃」は、当時の人々にとっては本当に正しい平均だったのかもしれません。
04なぜ「37℃」が残り続けたのか
- 有名な医師が出したキリのいい数字で、教科書や家庭に定着した
- 「37℃を超えたら熱」という目安が、わかりやすくて便利だった
- 測る場所(わき・口・耳)による違いが、あまり意識されてこなかった
基準そのものが間違いだったというより、時代と測り方によって"正解"が変わる数字だった、と言えそうです。

When doctors call 37°C (98.6°F) "normal," they are repeating a figure from Wunderlich, a 19th-century German physician who popularized temperature as a sign of disease. Modern data put the number slightly lower. In 2017, a team led by Harvard researchers examined roughly 240,000 measurements from 35,488 outpatients with no diagnosed infection.
The mean was 36.6°C, and 95 percent of individuals fell between 35.7°C and 37.3°C. Temperature declined a little with age, and each person seemed to have a characteristic baseline of their own.
