The Meteorological Agency issues advisories, warnings and, since 2013, emergency warnings for conditions expected only once in several decades. Municipalities issue evacuation instructions, and since 2019 both are mapped onto five numbered levels so that people can act without interpreting the words. Level 5 means the disaster is already happening. The phone alert sound is loud and cannot be turned down easily.
気象庁は注意報と警報を出し、2013年からは、数十年に一度と見込まれる状況に特別警報を出しています。避難の指示は市町村が出し、2019年からはどちらも五段階の警戒レベルに対応づけられ、言葉を読み解かなくても動けるようになりました。レベル5は、すでに災害が起きているという意味です。携帯に届く警報音は大きく、簡単には下げられません。

Anyone staying in Japan for long will eventually have their phone scream at them, and it helps to know what the words mean. The Meteorological Agency runs three tiers. An advisory means conditions that could cause damage. A warning means serious damage is expected. An emergency warning, added in 2013, means a scale seen perhaps once in several decades, and the agency says plainly that lives are at risk.
Separately, municipalities issue evacuation information, and because people hesitated over which phrase outranked which, the two systems were mapped onto five numbered levels in 2019. Level three means older people and anyone slow to move should leave; level four means everyone in the designated area should leave; level five means the disaster is already under way and you should do whatever is safest where you are, which may mean moving upstairs rather than outside.
The word evacuation also does not only mean a shelter: going to a solid building nearby, or a friend's house on higher ground, counts. Hotels will usually tell foreign guests what to do, and the agency publishes its warnings in several languages.
日本に長く滞在すれば、いずれ携帯が鳴り叫びます。言葉の意味を知っておくと役に立ちます。気象庁の段階は三つです。注意報は、災害が起こるおそれがあるという意味。警報は、重大な災害が起こるおそれがあるという意味。2013年に加わった特別警報は、数十年に一度と言われる規模で、命に危険が及ぶと気象庁がはっきり言います。
これとは別に、避難に関する情報は市町村が出します。どの言葉がどちらより重いのかで人がためらうことが分かり、2019年に両方が五段階の警戒レベルに対応づけられました。レベル3は、高齢の人や動くのに時間のかかる人は避難する段階。レベル4は、対象の地域の全員が避難する段階。
レベル5は、すでに災害が起きており、その場で最も安全な行動を取る段階です。外へ出るより上の階へ移るほうがよいこともあります。避難とは避難所へ行くことだけを指しません。近くの頑丈な建物や、高い場所にある知人の家へ行くことも避難です。宿は外国からの客に指示を伝えてくれますし、気象庁は警報を何か国語かで出しています。

The emergency alert broadcast, used for earthquakes, tsunami, and municipal evacuation instructions. It overrides silent mode by design, and it is not a scam or a test unless it says so.

It is not legally compulsory in Japan, which surprises people. It is a strong instruction from the municipality and should be followed, but nobody will force you.
