The My Number system is often misunderstood, so it helps to separate two things. The number itself is assigned to every resident, including foreign residents, and you cannot opt out of it; it links your tax records and social security. The card is separate, and applying for it has always been voluntary.
For years take-up was low, because people did not see what it was for and there had been public concern about data leaks. That changed when the government tied it to health insurance. Since December 2024 new paper insurance cards are no longer issued, and hospitals read the card at a face-recognition terminal instead.
People without one receive a certificate of eligibility instead, so nobody is cut off, but the practical pressure to get the card is obvious. It also serves as photo ID, which matters in a country where many adults have no driving license.

Yes. Anyone with a resident registration gets one, and you can apply for the card on the same terms as a Japanese national. Your employer will ask for the number when you start work.

QIs the card compulsory now?
Legally no. But paper insurance cards are no longer issued, so without the card you have to use a certificate of eligibility instead. In practice most people find it easier to get the card.

No. The chip holds an electronic certificate, not your records. The card identifies you so the hospital can look up your insurance status online; the data lives in the system, not in your pocket.
