Since December 2024 new paper health insurance cards are no longer issued, and the My Number card has taken over that role, which has pushed take-up sharply.

People without one receive a certificate of eligibility instead, so nobody is cut off, but the practical pressure to get the card is obvious. It also serves as photo ID, which matters in a country where many adults have no driving license.

AYes. Anyone with a resident registration gets one, and you can apply for the card on the same terms as a Japanese national. Your employer will ask for the number when you start work.
外国籍の住民にもマイナンバーは割り当てられますか。割り当てられます。住民登録のある人は全員です。カードの申請も日本国籍の人と同じ条件でできます。働き始めるときに、勤め先から番号を聞かれます。
QIs the card compulsory now?
ALegally no. But paper insurance cards are no longer issued, so without the card you have to use a certificate of eligibility instead. In practice most people find it easier to get the card.
いまはカードが義務になったのですか。法律の上では義務ではありません。ただ紙の保険証が発行されなくなったので、持たない人は資格確認書を使うことになります。実際には、作ったほうが楽だという人が多いです。
QIs my medical history stored on the card?
ANo. The chip holds an electronic certificate, not your records. The card identifies you so the hospital can look up your insurance status online; the data lives in the system, not in your pocket.
カードに病歴が入っているのですか。入っていません。チップにあるのは電子証明書で、記録そのものではありません。カードは本人を示すもので、保険の資格は病院がオンラインで照会します。情報はあなたのポケットではなく、仕組みの側にあります。

MYTHJapanese people can refuse to have a My Number.日本人はマイナンバーを持たない選択ができる、と思われがちです。
FACTThe number is assigned to every resident and cannot be declined. Only the physical card is optional, and since paper insurance cards ended, that choice has become harder to make.番号は住民全員に割り当てられ、断ることはできません。任意なのはカードのほうだけで、紙の保険証が終わったことで、その選択もしにくくなりました。

豆知識Background
マイナンバー制度では、平成27年（2015年）10月から住民票のあるすべての人に12桁の番号が通知され、平成28年（2016年）1月に利用が始まった。カードの交付は任意だが、令和6年（2024年）12月2日に従来の健康保険証の新規発行が終わり、既存の保険証も最長で令和7年（2025年）12月1日まで、という移行が組まれた。カードを持たない人には資格確認書が申請なしで交付される。カードのICチップには電子証明書が入っており、コンビニでの住民票の取得や、確定申告の電子申請にも使える。
